多科协作，续命接力！城口县妇幼保健院成功抢救一例妊娠合并心衰高危孕产妇

font-style: normal; font-weight: 400; line-height: 1.8; letter-spacing: 2px; box-sizing: border-box;" powered-by="xiumi.us"><p style="white-space: normal;margin: 0px;padding: 0px;box-sizing: border-box;"> 4月30日，刚结束查房的李巧玲科主任接到门诊医师急诊电话“有一孕妇呼吸困难，需要协助诊疗”，李主任立即到场查看，发现该年轻孕妇面色蜡黄、口唇发绀，呼吸快。追问其已25岁，既往有结核、哮喘病史，未规律诊疗。前一胎为剖宫产，现第二胎妊娠32+3周，已出现胸闷、憋气、呼吸困难5天，无法平卧、夜间无法入睡，近1天症状加重，来院就诊。李主任经初步检查考虑为“妊娠合并心衰、呼衰”，病情非常危重，立即将孕妇推送入病房，持续吸氧、胎心监护及心电监护，并通知医务科、护理部、检验科、麻醉科，心内科医师等到场成立抢救小组，同时联系市妇幼重症医学科张教授，寻求远程指导。</p><p style="white-space: normal;margin: 0px;padding: 0px;box-sizing: border-box;"> 经过综合查体及相关辅助检查，多名医师进行了多次探讨，考虑在继续待产过程中孕妇有随时发生心力衰竭、肺梗死或者心律失常而死亡可能，建议快速稳定病情后立即终止妊娠为宜，帮助缓解孕妇心肺负荷。</p><p style="white-space: normal;margin: 0px;padding: 0px;box-sizing: border-box;"> 孕妇虚弱的样子揪起了在场每一位医护人员的心，大家全都绷紧神经，有条不紊地开展抢救治疗，在给予洋地黄、呋塞米等药物（强心、利尿）处理后，孕妇呼吸困难等症状稍缓解，积极完善相关检查及术前准备，送入手术室急诊行剖宫产。在心内科医师、麻醉科医师、产科医护团队的携手配合下，手术顺利进行。一声新生儿的啼哭成为当下最美好的声音，产妇平安分娩一体重约1625g新生儿，转与新生儿科医师进一步诊疗。在我院产科医疗团队的娴熟配合下手术顺利完成，但这仅仅只是成功的第一步，接下来更大的难关是术后的72小时，心衰病情加重的危险时期，产妇随时有死亡风险。</p></section><section style="font-size: 16px; font-style: normal; font-weight: 400; line-height: 1.8; letter-spacing: 2px; box-sizing: border-box;" powered-by="xiumi.us"><p style="white-space: normal;margin: 0px;padding: 0px;box-sizing: border-box;"> 同时，在接到我院求助电话后，市妇幼重症医学科张耀宗教授立即驱车赶往，历经5+小时车程来到我院，未经休息，直接与李巧玲科主任于产妇病房碰面，查体分析目前产妇病情及诊疗。对于该产妇，他指出“现仍不能松懈，术后这72小时才是最艰难的，尤其是24小时内心衰、肺梗死、心律失常死亡高发”，与李巧玲科主任探讨并制定了其术后相关治疗。安排专人护理，每1小时测尿量，每2—4小时用药强心、利尿，严密监测其心率、呼吸、血压等生命体征，历经20+小时，产妇终于度过了最危险的时刻；术后第一天，张耀宗教授查房指示，现产妇病情平稳，需转至综合医院进一步诊疗。与其家属沟通后，在张耀宗教授及李巧玲科主任亲自护送下，安全送达市妇幼重症医学科。</p><p style="white-space: normal;margin: 0px;padding: 0px;box-sizing: border-box;"> 经历数天，在多科、多院、多名医护人员的协助诊疗下，产妇病情终于得以控制。市妇幼重症医学科分享了她此刻的照片，看到她微笑的那一刻，我们的心才真正的放下了.......</p></section><section style="font-size: 16px;
