城口县打造医共体集团医院 让优质医疗资源县域共享
<p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">近年来，我县以“医通、人通、财通”为主线，补齐基层医疗卫生短板，推动县域内医药卫生体制综合改革。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">改革的第一步，便是组织改革。针对全县医疗卫生机构规模小、体量小、人员少的实际情况，我县盘活优化医疗卫生服务资源，推动融合发展，于2021年3月建立了“1+4+N”城口县医共体集团医院，构建了集团医院“三通”新格局。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;"></p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">“其中1是指医共体集团医院，4是集团的4家牵头医院，包括县人民医院、县中医医院、县妇幼保健院和县疾控中心，N则是指24个基层医疗机构和125个村卫生室。这4家县城牵头医院和所有基层医疗机构都纳入集团医院一并管理。”城口县卫健委相关负责人介绍。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">据了解，城口县的医疗机构在横向和纵向方面都建立了联系，以此促进集团医院的一体化。在纵向方面，以专科专病联盟的方式，建设了以县人民医院、县中医医院、县妇幼保健院为牵头单位的纵向“基本医疗团队”和以县疾控中心、县妇幼保健院为牵头单位的纵向“公共卫生团队”。在横向方面，县级医疗卫生机构之间横向联合，按照各自不同定位实行“错位发展”。同时，乡镇以中心卫生院辐射，带动周边乡镇卫生院的方式实现横向联合，进一步落实基层首诊制。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">“最初到明通中心卫生院治疗，医院多次建议转上级医院进行手术，但是去县里开销大，我就一直没有就诊。”67岁的邓先生在2年前检查身体发现右侧腹股沟包块突出，加重伴疼痛半年余，但由于家庭经济原因一直没有进行治疗。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">今年9月，邓先生听说县人民医院研究这方面的专家医生到明通中心卫生院来支援，就抱着希望来医院外科就诊，最终检查确诊为右侧腹股沟可复性斜疝，并成功入院完成了治疗。这次治疗是通过医共体工作推进，开展的首例手术治疗，不仅使邓先生在基层医疗机构也享受到了县城医院的同质化治疗服务，也进一步加强了医院间的互联互通，促进基层医疗机构服务能力提升。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px;"></p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-align: justify; text-indent: 2em;"><span style="margin: 0px; padding: 0px; border: 0px; text-indent: 2em;">为打造共享县域内医疗优势资源，形成服务共同体，城口县不断推进病种下沉、公共卫生服务下沉、药事服务、服务质量同质化、县域内医疗资源共享、双向转诊服务和医防融合发展工作。</span></p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">自改革以来，城口县开展了全县基层医疗机构66个病种诊疗能力调查，开展培训讲座约20场次。完善了县乡两级药品目录，实现县乡两级药品目录共享，对下级药品衔接工作开展情况进行督导，确保下转病人疾病诊治连续性用药需求。县级牵头医院完成了对23个乡镇的业务指导工作，开展义诊活动23场次，累计服务基层群众3570余人次；推广中医适宜技术4类10余项，培训医务人员187人次，完成11个基层医疗机构等级创建进度的全面督导。此外，城口县还建立了远程医疗平台，截止目前，2022年以来城口县与重医附一院开展远程动态心电图814例、远程动态血压86例，与重庆大学附属三峡医院开展远程影像诊断179例，与乡镇卫生院开展远程影像诊断795例，初步实现了“乡镇检查、县级诊断和县级检查、市级诊断”的远程诊疗格局。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;"></p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">“我们不断实现医通的目的，就是为了推动医疗资源的下沉，让县城医疗带动乡镇医疗的发展，使老百姓能够在当地就能就诊治疗，不用舟车劳顿跑到县城或者是主城区接受治疗。”县卫健委相关负责人向记者娓娓道来。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">根据城口县卫健委提供的数据看，显然“医通”的不断推进有效提升了患者在基层医疗机构就诊的获得感。2021年全市各区县中，基层总诊疗人次占比最高的为城口县，区域内总诊疗人次中有64.91%人次在基层医疗机构进行诊疗。2022年上半年，随着双向转诊流程的不断完善，向基层医疗机构转诊患者同期增长11.4%；基层诊疗率达到68.2%，县内就诊率达到90.17%。</p><p></p><section class="_135editor" data-role="title" data-tools="135编辑器" data-id="118510" style="margin: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">“县人民医院作为我县首家编制备案制工作试点公立医院，今年启动了首批62名编制备案总量补充人员招录工作，目前入编11人，其中2人分别定点安排至明通镇和双河乡卫生院工作。”县卫健委相关负责人说，“今年我们招聘的这两名人员虽然在乡镇工作，但编制在县人民医院，服务期最少满一年。”</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">这是城口县为解决基层医院“人才荒”的问题，所实施的“县聘乡用、乡聘村用”制度。基层各成员单位根据需求提出用人需求计划，由集团医院党委会同人社部门按规定程序统一组织招聘，人员身份纳入集团医院编制管理，招聘后分配到基层成员单位。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;"></p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">基层医院留不住人的问题一直制约着县域医共体“三通”建设的发展，人才流通的壁垒，直接造成医疗资源结构不合理。对此，城口县通过加强人才队伍建设、建立高级职称评定激励机制、加强人员能力提升、推动医疗机构管理规范化、完善基层医疗机构绩效管理等方式，不断扩充基层医院的人才队伍，提升城区医院医务人员下基层的意愿，以此提升基层医院医疗服务水平。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">“为了稳定乡镇卫生院医务人员队伍，我们开办卫生院食堂、落实艰边津贴、畅通职称及职务晋升渠道等优惠政策，就是为了让医务人员安心在基层，奉献在基层，创业在基层，服务在基层，实现群众常见病和多发病看病就医在基层。”县卫健委相关负责人说。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">据了解，今年城口县从县城的医院里派选了5名有资格晋升副高职称的医务人员到基层服务1年；对部分没有副职的乡镇卫生院推荐符合条件的人员12人；选派参加市级骨干医师、重性精神科医师转岗等培训20人；选派6名基层医疗机构人员参加中医馆骨干人才培训，6名中医技术骨干参加了中医急诊急救知识培训；全县继续医学教育培训开展30余场次，参加培训人员2000余人次。</p><p></p><section class="_135editor" data-role="title" data-tools="135编辑器" data-id="118510" style="margin: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">为加强疫情防控，预防接种安排，我县在26个新冠疫苗接种点配备齐了除颤仪、呼吸机、心电监护仪等急诊急救设备39台件。“但我们使用‘资金池’内的资金集中统一采购，节约了60多万元。”县卫健委相关负责人说，“今年东安镇卫生院业务用房改扩建建设用地土地征收费也是使用的‘资金池’内的资金”。</p><p style="margin-top: 0px; margin-bottom: 0px; padding: 0px; border: 0px; text-indent: 2em; text-align: justify;">“资金池”正是城口县县域医共体中“财通”的一部分。财政每年通过统筹整合部分医疗卫生和计划生育专项资金注入“资金池”， 每月按一定比例提取集团医院各成员单位医疗服务收入。“资金池”内资金主要用于集团成员单位重点学科建设、基础设施建设、基础设备购置、人才引进等方面。 截至目前，城口县财政统筹整合医疗卫生专项资金注入“资金池”545.9万元。</p><p style="margin-top: 0px;
